Club Name Badges are ordered separately from the membership forms, at a cost of $8.50 each.  Click on the form below for a printable copy.  Please note:  Badge order forms must be accompanied by  CHECKS ONLY, payable to Carolyn Whitehead.  Forms/checks should be placed in the Computer Club mailbox up at SAGE Center.  
PLEASE NOTE: We wait to submit orders until we have orders for at least TWO badges.  Our supplier will add additional charges if we only order a single badge.
